On this Give to the Max Day, Vail Place has a special request on behalf of those living with serious and persistent mental health challenges and that is to give generously if you can to help those that we serve to be able to empower them to have new opportunities to live out their dreams. 


VAIL PLACE | WHO WE ARE 

Since 1981, Vail Place has provided community-based mental health support services throughout the Twin Cities area. We serve as a single point of access and coordination for individuals who want and need support throughout their recovery journey. These supports include case management, housing, employment opportunities, health education, health care system navigation, safe relationships, and community connections. At Vail Place we serve over 4,000 clients annually. Our person-centered services make a measurable difference in the lives of our clients and community. 

PROGRAMS AND SERVICES WE OFFER

Clubhouse Community Support Program

Our organization was founded on the values and principles of the internationally acclaimed Clubhouse Model. Clubhouses are designated as an Evidence Based Practice by SAMSHA, and they support a wide range of needs: providing employment and housing opportunities, social connectedness, skill building, health education and support, system navigation, and community-based wellness resources. Vail Place currently has the only two (2) accredited Clubhouses in the state of Minnesota and is recognized by Clubhouse International as a pioneer in the Clubhouse movement nationwide. We serve over 750 individuals annually through over 22,000 visits. Plans to open a Ramsey County location in 2024 are underway.

Our members tell us again and again that “our Clubhouses are unique, they saved my life, and they offer a resource that simply is not present anywhere else”. Give to the Max Day plays a critical part in the story of so many this year helping us protect these programs. 

Our Clubhouse programs support a wide range of needs: providing employment and housing opportunities, social connectedness, skill building, and supporting overall health and wellness. 

Targeted Case Management (TCM)

Adult Mental Health Targeted Case Management services at Vail Place are provided with an inclusive, trauma-informed, and person-centered focus. Our case managers help individuals navigate the complexities of the social service, mental health, and health care systems. Our team is one of the largest in the state and we serve over 1000 individuals annually through over 2800 visits. 

Community Health and Value Based Programs

Assertive Outreach and Stabilization (AOS) and Vail Connect

AOS and Vail Connect are integrated care models designed by Vail Place to engage individuals in crisis experiencing mental illness, chronic substance abuse disorders, and a variety of social and health related challenges. Our goal is to help stabilize people’s lives in the community environment which improves patient satisfaction, reduces healthcare costs, and improves overall population health. We work in partnership with BCBS, North Memorial, HealthPartners, and UCare in our value-based community health programs.

Housing Program and Services

Vail Place has offered housing support for many years to help people find safe and affordable homes. In 2021 Vail Place created a Housing Division, which brings all our individual housing programs and services under one umbrella, allowing for greater integration, utilization, and leveraging of resources to better meet our mission. We offer site specific and scattered-site housing services that best meet the unique needs of all persons served.
