Granite Falls Living at Home Block Nurse Program

Granite Falls Living at Home Block Nurse Program (GF-LAH/BNP) is dedicated to providing seniors essential services to help them live well and safely in their own homes while maintaining a good quality of life. Utilizing a team of trained volunteers, our services and programs are offered at no cost to seniors and people living with disabilities. We provide services such as in and out of town assisted transportation, friendly visits, short term respite care, errands and light chore service.  We also provide meaningful social engagement through programs such as the H2H memory cafe, the BB memory choir, gentle fitness exercise classes, caregiver and diabetic support groups.

Founded in 2000, the Granite Falls Living at Home Block Nurse Program (LAH/BNP) is a community based service provider organization that supports elderly residents, helping them to maintain their quality of life.

The Granite Falls Living at Home/Block Nurse Program is known throughout the community as a compassionate, reliable, and trustworthy resource for seniors and the disabled. We provide essential health and social services to seniors and people living with disability, in the Granite Falls community, enabling them to stay in their homes, avoiding premature entry into the nursing home setting.  In addition to providing direct services, our program provides information about other local and regional health and humans service provider organizations, helping clients to navigate the sometimes confusing service options.  Many seniors depend on our program.  Our track record demonstrates how we meet our mission of helping the elderly to live safe and independent in their own homes.
