Started in 2014, Age Well Arrowhead is a non-profit located in Duluth Minnesota, focused on connecting older adults and their caregivers to services that support independent and healthy aging. By building a structured network of experienced team members, local organizations, and community volunteers, Age Well Arrowhead has been able to provide over 20,000 hours volunteer-based services to 525 clients in Duluth and its surrounding communities. Its Groceries-to-Go program, started in 2016, has processed and delivered more than 3,500 grocery orders to home bound seniors. Knowing the benefits of preventative coordinated support, Age Well Arrowhead has worked over the past six years to build the only community-based non-profit network supporting independent aging in the arrowhead region. Through Age Well Arrowhead, older adults and their caregivers can access volunteer-based services, care consultation (LWAH Risk Screens, customized care plans, and resource identification), caregiver counseling (risk screens, respite options, community supports, and family meeting facilitation), and evidence-based trainings. Additional services include homemaker, chore, transportation, companionship, and grocery shopping and delivery. Provision of these services has led to Age Well Arrowhead receiving the Charities Review Seal of Approval and to being certified by Points of Light as a Service Enterprise Volunteer Program. Referral relationships have been established with local health care providers, county staff, home care agencies, aging network service providers, parish nurses and churches to identify older adults and caregivers who may benefit from Age Well Arrowhead’s services and supports. Age Well Arrowhead is a trusted referral partner with Essentia Health, the largest health care provider in the region. As this referral network has been developed and community awareness of Age Well Arrowhead has grown, demand for services has steadily increased.
