Trellis provides services, connections and innovations to help people thrive as they age. Through partnerships with community-based organizations, healthcare organizations and others, we deliver services where they are needed most. We are committed to reducing disparities in access and ensuring equity and inclusivity. Trellis serves people across Minnesota and, in some of our services, people across a broader swath of the nation.

As the designated area agency on aging for the Twin Cities metro area, we fund community-based organizations that provide services to older adults in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington counties. 

In partnership with the Minnesota Board on Aging, Trellis provides Senior LinkAge Line services connecting thousands of older adults each year to services that keep them healthy and engaged in their homes and communities. 

Juniper, a program of Trellis, is a network of community-based organizations and health systems that makes evidence-based health promotion programs available to people throughout Minnesota. Together we help people live healthy and connected lives.

Trellis Pension and Retirement Rights provides legal advice to help people claim the dollars they are owed. During the past 10 years, Trellis Pension and Retirement Rights has served more than 2,000 people and recovered more than $1.5 million.

Our Dementia Friendly Communities Initiatives work to make communities places where people with memory loss can live and thrive. ACT on Alzheimer’s® (actonalz.org) equips communities to support people with Alzheimer’s disease and other dementias. It has fostered more than 60 community-based Action Communities across Minnesota. Stratis Health, Alzheimer’s Association of Minnesota and North Dakota, Minnesota Board on Aging and Trellis are core members of the collaborative. Dementia Friends is a global movement to change the way we think, act and talk about dementia. The program has trained more than 23,000 Dementia Friends. The Remember Project (rememberproject.org) uses the arts as a tool to support the Dementia-Friendly Communities movement by raising awareness and ending the stigma, fear and isolation related to memory loss.

Trellis strives to meet the opportunities and challenges of an older population with new solutions and partnerships. We are leading the charge on integrating health and social care to produce better life and health outcomes — and doing so in a way that delivers crucial service while containing exploding costs.
