Trellis provides services, connections and innovations to help all people thrive as we age. Through partnerships with community organizations and healthcare organizations, we work to bridge social care and medical care to produce more positive outcomes for individuals and communities. We are committed to reducing disparities in access and ensuring equity and inclusivity so that all people thrive as we age.

We have decades of experience developing powerful partnerships and creative approaches to help people age with good health and well-being, including:

• Advocating with and for older adults to ensure better health, income security, and increased resources for our organizational partners across Minnesota.
• Launching the Juniper community care hub to help people take charge of their health and well-being through evidence-based programs and social interactions.
• Establishing the first statewide dementia friendly community initiative in the United States.

Together, with your philanthropic support, we make it possible for older adults to thrive and develop the capacity of communities to care for our rapidly aging population.
