Southside is a 501(c)(3) nonprofit, Federally Qualified Health Center (FQHC) providing affordable access to health care for residents of south Minneapolis and surrounding areas, regardless of their ability to pay. Founded in 1971, Southside has a history of 54 years of serving the diverse community of south Minneapolis and is deeply rooted and committed to growing our capacity to adapt and respond to the needs of the people we serve. 

Southside’s principal activities include providing person-centered, high-quality, culturally responsive primary and preventive care including medical, dental, vision, integrated behavioral health, and community outreach, and helping to connect patients to community resources that address their health-related social needs. In addition to traditional health services, we also provide Health Care Home coordination, chronic condition management, insurance eligibility assistance, care coordination, interpreter services, nutrition counseling, cooking classes, maintain a community garden and more. 

In April 2026, Southside will open a brand-new building, “One Southside” on East Lake Street, a major thoroughfare in south Minneapolis. One Southside will increase access to serve 18,000+ unique patients, bring all primary care services under one roof. One Southside will have more space for patient care, the option to have evening and weekend hours, additional services like x-ray and mammography, flexible community space, and a design that focuses on trauma-informed and healing principles, encourages collaborative visits, and offers an exceptional patient experience that matches the award-winning quality of care that people receive at Southside. One Southside will serve as a hub for the community to get the support they need to achieve their best health and wellbeing. This holistic, person-centered approach is our way of promoting health equity by giving people the care that they need and deserve.

For the past seven years, Southside has been the only Minnesota health center to earn the federal government’s Gold Quality Leader Award for clinical quality outcomes in the top 10% among the 1400+ community health centers in the U.S. In 2024, Southside received National Quality Leader awards for our Heart Health and Diabetes outcomes, and top recognition for Preventive Health, Improving Access to Care, High Value Care, and Advancing Health IT. Southside’s 98% patient satisfaction score and 87% staff satisfaction score support the sustainability of our organization thanks to the demand in the community for the high-quality care we provide, and the continuity of the staff needed to provide the care. Our achievements related to excellent patient and staff experience and exceptional clinical quality have made us known as a reliable and effective community partner for funders. Southside has been a Minnesota Department of Health (MDH) Health Care Home (HCH) since 2013 and certified at the highest Level 3 providing person-focused, high quality, culturally responsive care.
