Open Circle Adult Day Services

Open Circle is a community-based adult day program that has been serving families for many years. Participants come to Open Circle for many reasons: dementia, isolation, chronic care management, mental health, and post acute. Open Circle offers comprehensive programming to engage participants in activities that support their identity and provide purpose for each day. Open Circle offers care management, on-site physical and occupational therapy, and assistance with personal care in a secure environment.
