The mission of Nokomis Healthy Seniors (NHS) is to enhance the lives of our community elders by helping them continue to live independently in the homes they love. NHS is a nonprofit Living at Home/Block Nurse Program that has served the greater Nokomis neighborhood of Minneapolis since 1994.

We do this by offering affordable, high quality health services and coordinating volunteer assistance and community resources to serve older adults in our community. These services are more vital than ever, due to rising health and home care costs and an increasing older population.

Nokomis Healthy Seniors’ services have evolved to meet the needs of today’s older adults. We help our neighbors to continue living safely and independently in their homes by:

  • Providing information and referral to community services
  • Assisting in the coordination of services
  • Recruiting and coordinating volunteers to provide services
  • Providing nursing case management/health care coordination

One strength and distinction of NHS is that our programs are free or low cost, making them accessible to most individuals. And, our programs address and support the physical, medical, and social aspects of seniors’ lives.
