Horizon Health was established in 1991 to provide resources for seniors who want to remain living in their homes and communities for as long as possible. Since then, we are staffed with over 150 healthcare professionals who provide services to 300 people on a daily basis. We have grown to offer 13 facilities throughout Morrison County and the City of Brainerd including assisted living, supported living, and independent living communities, along with adult day services. People with disabilities appreciate the Horizon Health resources including in-home rehabilitative services. Our residents with memory challenges receive individualized care from our team members who are trained to provide services to those with dementia. Horizon Health's Faith in Action program coordinates volunteers who build ramps, install grab bars in homes, and drive those who are homebound to doctor appointments. Your financial gifts support our efforts to reimburse mileage expenses incurred by the volunteer drivers. Horizon Health Home Care and Hospice divisions help people age with dignity, and find comfort and peace when terminally ill.
