Every day, seniors in our community are struggling—with rides they can’t arrange, homes that aren’t safe to move around in, or long days without a friendly voice. Through Faith in Action, Horizon Health connects these neighbors with caring volunteers who show up, step in, and make life better.

Our volunteers build ramps, install grab bars, provide rides to essential medical appointments, and offer simple, heartfelt services like friendly visits, phone calls, light mental health support, and much-needed respite breaks for family caregivers.

Horizon Health: A Full Circle of Care

Faith in Action is just one part of the larger Horizon Health family.

Since 1991, we’ve been dedicated to helping seniors and people with disabilities live independently, with dignity, for as long as possible. Today, over 150 Horizon Health professionals serve more than 300 people every day across Morrison County and surrounding areas—including Pierz, Brainerd, Little Falls, and extending all the way to Long Prairie.

Our care extends across 13 facilities offering assisted living, supported living, independent senior apartments, and adult day services. We also provide in-home rehabilitative care and compassionate, individualized support for those facing memory challenges.

Through our Home Care and Hospice services, we walk alongside individuals and families as they navigate aging, serious illness, and end-of-life care—helping people find comfort, peace, and dignity in their most tender moments.
