Hearth Connection uses innovation, research, and best-practices to end long-term homelessness. Our mission is to build relationships, resources, infrastructure and understanding so that no one in Minnesota remains homeless, and people who were homeless can prosper.
Hearth Connection works with a broad spectrum of partners to offer housing and support to over 1,200 people each year. Our regional supportive housing collaboratives cover 34 counties.
People who participate in the supportive housing collaboratives are long-term homeless families, adults, or unaccompanied youth—many of whom experience significant health issues, mental illness, substance abuse, or trauma from years on the streets. With the help of our service partners, participants develop individualized goal plans and work towards long-term stability, skills, and recovery. Data from 2012 shows that despite having been homeless an average of 7 years, 71% of participants are in permanent housing within two months of enrollment.
At Hearth Connection, we know the value of collaboration in accomplishing seeming insurmountable tasks. In the last decade, we changed the response to long-term homelessness and proved that people with considerable barriers to stability can maintain housing and transform their lives.
