Chisago Age Well Coalition

The Chisago Age Well Coalition is a recognized 501(c) (3) not-for-profit.  We are volunteer run group of more than 30 local community members and service providers, both private and non-profit, who work together to benefit older adults and their caregivers in Chisago County.  Chisago Age Well was created in January 2018 as the result of a statewide initiative called Silos to Circles. 

Our goal is to help older adults and their caregiver's live and age well in their home and community.

Our Mission is to increase awareness of, access to, and satisfaction with existing services and health resources in the community and to strengthen the relationships between senior community members, service providers, and health systems to benefit older adults and their caregivers.  

We rely entirely on grants and donations to provide free informative programs throughout Chisago County several times a year and the Age Well Expo held every October.

Open to everyone, our programs feature local experts speaking on topic of significance to older adults and their caregivers.  In addition to sharing a wealth of resources, Age Well events provide opportunities for older adults to gather and learn while they socialize and enjoy refreshments in a friendly, upbeat environment.
