Hospice: Caring for Those during their End of Life Journey


When individuals are diagnosed with a terminal illness, it is important that they are able to go through their end of life journey with dignity.  Hospice gives patients the opportunity to spend the last days in a caring environment with family.  Hospice is a program that not only provides support to the patient but also their families.  Patients are given the opportunity to have skilled nurse visits, hospice aide visits, chaplain visits, massage therapy, Healing Touch, and volunteers.  Hospice also has a bereavement program that is offerred at no charge to families, caregivers, community members, and friends.  CHI Health at Home provides comfort cares to terminally ill patients who reside in about a 60 mile radius of Albany including some of the following communities:  Avon, Freeport, Sauk Centre, Holdingford, Melrose, and Paynesville.  Hospice covers the costs of medications and DME Supplies (oxygen, wheelchairs, etc.) for comfort.  Bereavement services and supplies are not covered by Medicare or insurance, so hospice depends on donations to continue to service the local community. Please consider a gift to help those who have been diagnosed with a terminal diagnosis.  All donations stay local.
