"Bridging the Gaps Between the Community and the Resources They Need"

One of our most "publicly visible" programs is our Beyond the Fire Program. Helping victims of house fires is gut-wrenchingly  meaningful. Life as families know it has changed. After the fire is put out, that is where the connection to resources are needed the most. Many families simply don't know what they need and learn days later they are in need of assistance. In those moments they cannot even begin to imagine the lengthy journey ahead. We support and advocate for them every step of the way.  This is done with our partnerships with the American Red Cross (Minneapolis & St Cloud Chapters) serving  Anoka, Sherburne & Wright Counties providing case management and resource connections to help community members after a fire or natural disaster. 

CARE Resource Connection has other programs making measurable differences in lives by working directly with fire and police departments. These mobile integrated health programs are served behind the scenes, and make measurable impacts on a population health level, reducing 911 calls that save tax paying dollars. Our collaborations with health systems, healthcare providers, healthcare & mental healthcare professionals are dedicated to bridging these gaps, and creating equal access to care and services.  This is done by connecting individuals and families to the resources they need after repeated 911 calls for falls in the home, calls for an ambulance for a medical need, calls for patients that are at risk for nursing home placement, hoarding homes, and other high utilization 911 callers that are needing connections for care and services with community resources. We are proud to be paving the path and leading the way alongside the St. Louis Park Fire Department's Community EMT Program as we open services into Hennepin County.
