Care Partners: Provides support and companionship on the journey of
aging and serious illness. We engage trained volunteers and staff to provide practical help for older adults and the people who care for them so that they can stay in their homes and our community.

Services include:
• RN Care Coordination (palliative care)
• Caregiver coaching and support
• Dementia Education & Coaching
• Resource & Referral
• Advance Care Planning
• Volunteer companionship, respite and presence at end of life
• Senior Rides (volunteer assisted transportation)
• Senior Chore Program
