Care Partners Of Cook County

Care Partners of Cook County supports community members on the journey of aging and end-of-life. We engage trained volunteers and staff to provide practical help and companionship for older adults and the people who care for them, so they can stay in this beautiful corner of the world for as long as possible. We also support efforts to engage and educate the community to be age-friendly and age-wise, and advocate for a world that is welcoming and accessible to people of all ages. 

Services include:

•    Care coordination

•    Caregiver coaching and support

•    Dementia education and coaching

•    Resourcing and referral

•    Advanced care planning

•    Community classes, book groups, and workshops

•    Volunteer companionship, respite and presence at end of life

•    Volunteer assisted transportation (Senior Ride Program)

•    Volunteer help with home and yard work (Senior Chore Program)
